The Hyannis practice is located at 1070 Iyannough Rd. in Hyannis, between the DSW and Shaw's in the Festival at Hyannis plaza.

Parking is available directly in front of the practice.

Your primary care provider is your go-to resource for day-to-day health. They can also help you identify health conditions you may not have been aware of and manage those conditions through ongoing care.

    At One Medical Seniors, your health is our top priority. In order to help you thrive in every aspect of your life, our healthcare team and providers are committed to a comprehensive approach to your health. We’re here to listen, support, and find the best treatment plan for you.

  2. Your One Medical Seniors team is a group of healthcare professionals who provide team-based care in order to address every part of your health and wellbeing. We don’t just want to treat your symptoms. We aim to provide a comprehensive approach to your care. Your provider and your Health Coach work together to get to know you and what’s important in your health so you can get the most of your care with us inside and outside of the practice.

  3. A Health Coach is a part of your care team. You and the Health Coach work together to get to know you and what’s important in your health so you can get the most of your care with us inside and outside of the practice. The Health Coach will be there throughout the entire visit so that when you leave, they can set proper expectations and follow up accordingly.

  4. We accept Original Medicare with or without most Medicare supplement plans. We also accept select Medicare Advantage plans. To see if we accept your specific insurance plan, contact our Patient Enrollment team at 855-977-0652.

  5. If you’re an existing One Medical Seniors patient, you can make an appointment by calling the Current Patients phone number on your practice’s location page.

    To get started as a new One Medical Seniors patient, find a location near you and call the Become a Patient phone number to schedule your first appointment. Or, you can fill out this online form and one of our Patient Enrollment Coordinators will contact you.

  6. All One Medical Seniors practices are open Monday-Friday, and hours vary depending on the day. While we like to reserve a few dedicated hours throughout the week to coordinate with other doctors and specialists, as well as follow up with patients who require additional care, we are still available for our patients from 9 am to 5 pm during weekdays, should you need to give us a call.

    Current patients can call their practice at any time. A One Medical Seniors provider is available 24/7 for current patients via phone. We also offer same and next day appointments for current patients with acute needs

  7. When preparing for your first primary care visit at One Medical Seniors, make sure to bring the following health information:

    • All of the medications you take (this is preferred to a medication list), including inhalers, injections, supplements and over-the-counter medications
    • Any recent doctors’ notes or medical records you have
    • The name of your last primary care provider
    • Information on a recent hospitalization (if applicable)
    • Your insurance card

  8. Due to our team-based approach to care, One Medical Seniors patients choose one office to have as their home office. Certain One Medical offices outside of your home office may be out of network or otherwise unavailable to you. Find a location near you and call the Become a Patient phone number to schedule your first appointment. Or, you can fill out this online form and one of our Patient Enrollment Coordinators will contact you.

  9. If you are on Medicare with no secondary coverage, you can visit any One Medical Seniors provider. If you have a Medicare Advantage plan, check the list of insurances accepted by One Medical Seniors to make sure you are on a plan we accept within your area. If you are not currently on a plan we accept in your area, remember that the Medicare Annual Open Enrollment Period is your next opportunity to change your coverage.
